  1. A current passport valid for at least six months past the expected time of arrival in the U.S and any previous passports showing travel outside Sri Lanka
  2. Original DS-160 Non Immigrant Visa Application (NIV) form (Confirmation page of each applicant).
  3. The photograph must be 2″x2″ and have a white background. Non-compliant photographs will be rejected and the applicant will have to reapply
  4. Original birth certificate
  5. Bank draft for certified funds in the amount of US$160 or Rs.21,600 drawn in favour of “U.S. Embassy Colombo” for categories under B,C,D,F,J & M.
  6. Marriage certificate
  7. Children’s birth certificates
  8. Personal and business bank statements and bank books
  9. Deeds to any property owned
  10. Proof of employment (pay slip, EPF/ETF slips, tax returns)
  11. Proof of education
  12. Evidence of sponsor or host’s legal status in the U.S. (copy of passport, green card or visa);
  13. Financial statements of businesses owned and tax returns;
  14. Travel Itinerary with name of travel agent.
